Transfer Prescription(s)

Here are the steps to transfer your prescription to Sunflower Rx pharmacy:

1. Complete the form below, call or visit Sunflower Rx pharmacy to request an Rx transfer.

2. Give us the names of all the medications you want to transfer, along with Rx numbers.

3. Provide your current pharmacy’s contact information. We will contact your old pharmacy and take care of most of the process.

4. Wait for the transfer to be completed, allowing at least 1 business day.  Most transfers are completed the same day.
